Distinctive characteristics of this form:

 The redesigned form was printed after the last of the Type 10 designs had been printed in 1960 - well into the Australian Post Office period. It was a radical simplification of previous forms partly because at that time, telegrams were telephoned to the recipient and so the form was only used for those telegrams for which a copy was requested.
