Size of form overall:	127 × 202 mm.

Distinctive characteristics of this form:

has TELEGRAM in a central box (like the previous format) with four lines of text underneath;
has "NOT FOR TRANSMISSION" at the base referencing the Sender's name and address;
the main distinctions between the varieties are:
11A: the "TO" box has no separate provision for recording STATE;
11B: additional small box in the "TO" box to record the State of the recipient.

This was the first format for a transmission form issued during the Australian Post Office period.
