Characteristics:

  • 2 lines of vertical text at left with the second line extending 16 mm above the horizontal line;
  • the S of SENDING in the second line of the side text is slightly below the O of NOTE to the left;
  • no schedule number;
  • size of form is 178 × 215 mm;
  • size of date stamp box is 36 × 35 mm. and the form number is now centered above and almost outside the right edge of the box;
  • .
There were at least two printings of this form. The obvious difference is the position of the LH NOTE relative to the underline for the main heading. That line can align with the "sp" of "respectively" or with the "e" of "Office".

    (p1338/12 S 24.) in the top left corner - inferring this form was printed in 1912.

  • has a Stores Index No. 903 in the top left corner in lieu of a schedule number and also has "*30" above and left of the Form number at the right.
  • has two lines of vertical text moved to the top left corner. The line under the heading is level with "THE" at the left.;
  • size of form is about 180 × 187 mm;
  • size of date stamp box is 32 × 34 mm. and the form number is now centered above and inside the right edge of the box;
