Distinctive characteristics of this form:

window envelope;
form number (T.G. 65FA) and schedule number are printed under the right side of the flap on the reverse side;
first printed in 1966 and to be used concurrently with AA-GGE-3.

window with rounded ends, a glassine insert and no border;
