Heading and notes:	No form number. TASMANIA above Crest with ELECTRIC TELEGRAPH on either side of the Crest. URGENT. printed vertically in orange.
Message area:	7 lines.
Reverse side:	Blank.
Colours (text & form):	Black on light grey.
Size of form overall:	169 × 215 mm.

Distinctive characteristics of this form:

has the same basic format as for TC-TO-3C which was used during the early 1880s.

This is the only known example of a Tasmanian urgent rate transmission form from the Colonial period.
